Police Reports
Police reports may only be released to authorized parties and according to State law. There are reports involving certain types of crimes that cannot be released to anyone except to the district attorneys or law enforcement agencies.
CARFAX Police Crash Assistance Program
The Daly City Police Department is now participating in the CARFAX Police Crash Assistance Program. All collision reports for the Daly City Police Department are now available to purchase online via CARFAX. You can access the collision reports by visiting the CrashDocs website.
Credentials
To obtain a copy of your police report, you must be either:
- A victim or a named involved party to the case (not merely a witness).
- An attorney, an authorized agent, or insurance representative. An attorney or authorized agent must include a signed authorization from the involved party. An authorized agent must also include a copy of the involved party's identification. An insurance representative should reference the name of the insurance company and the insured party's information.

Please fill out a Report Request Form (PDF) and send the completed form and a copy of your identification with a self-addressed stamped envelope to:

You can also drop off the form at the Police Department's front desk during operating hours (Monday through Friday, 8 am to 6 pm, Saturday, 8 am to 12 pm).
You can also send the completed report request form with a copy of your identification as an attachment via email to the Records Division. When sending a request via email, please enter "DCPD Report Request" in the subject line.
Feedback
Your request will be processed and it will be determined if the report is releasable to you. If releasable, a copy of the report will either be mailed via postal mail, or upon your request, you will be called when your report is ready for pick up. You will also be advised if the report is not releasable to you.
